Without it, the two faces of a double-sided card show through each other and the images overlap; and any artwork printed without a white-ink backing comes out semi-transparent.

A double-sided colour transparent card must have a white-ink backing — leave it off and the two faces overlap and show through one another. When you design one, plan around where the front and back artwork line up, so a layout clash does not cause a printing error.
